Myself Tarak Nath Maity having UAN No.-100389493385, DOB 13.09.1969 and Mobile No. 9051542969 but in EPF organisation it is registered as Tarak Maity, 13.04.1960 and 9339299710. Joint Declaration has been given to Kolkata regional office on 12.12.2017 1nd 31.05.2018 respectively but on telephone inquiry I found that the correction has not done.
